History

A 74-year-old with a history of cardiac disease with an AICD and a recent history of low grade fever developed acute low back pain after bending over to pick something up. The initial imaging assessment utilized lumbar spine radiographs, which were nonspecific. This was followed by a lumbar spine CT (Figures 1-3). Because of the patient’s AICD he was unable to undergo an MRI. Hand findings and the patient’s medical history (not described) prompted the patient to undergo hand radiographs (Figure 4).
